The purpose of the Quarterly Progress Report is to provide First 5 Ventura County with a summary of your program's accomplishments each reporting period.

Line Item Transfer Request Form

Submit this form to request any change to the contract budget (Exhibit C) exceeding 10 percent. You must receive prior approval before making any change to the contract budget that exceeds 10 percent. (As stated in the contract, you may transfer funds between the line items, if such transfers represent less than a 10 percent change to that item.)
